Recurrent Throat Infections in Older Adults: A Chronic Tonsillitis Case Report

Abstract

This case study focuses on a 64-year-old male client, Mr. M, from Narag village, Sirmour district, Himachal Pradesh, diagnosed with chronic tonsillitis. The primary objective was to assess his health condition, identify contributing factors, establish accurate nursing diagnoses, and implement comprehensive, evidence-based nursing care to promote recovery, prevent recurrence, and enhance community-level health awareness. Data were collected through interviews, physical examination, family assessment, and environmental appraisal. The client reported a persistent sore throat, difficulty swallowing, hoarseness, and mild fever. Examination revealed enlarged, reddened tonsils with exudates and tender cervical lymph nodes. His history indicated recurrent infections, often aggravated during winter and after consuming cold foods. Family history revealed similar episodes in his son, suggesting a possible hereditary predisposition and shared environmental exposure. Predisposing factors included frequent cold exposure, poor hydration, inadequate oral hygiene, and delayed treatment. Environmental assessment noted poor ventilation and smoke exposure from traditional cooking. Nutritional evaluation indicated an irregular diet and low intake of warm, nutrient-rich fluids. Nursing diagnoses based on NANDA-I were: acute pain, impaired swallowing, risk for infection, and deficient knowledge regarding disease management and prevention. Interventions included warm saline gargles, hydration, rest, soft diet, oral hygiene maintenance, health education on diet and hygiene, and environmental modifications. Following care, the client demonstrated marked improvement reduced pain, easier swallowing, normalized temperature, and increased understanding of preventive measures. This study underscores the vital role of community health nurses in holistic management, and education for chronic tonsillitis, highlighting impact on improving health outcomes and quality of life in rural populations.
