Congenital abnormalities of the female reproductive system are known as Mullerian duct anomalies out of aberrant Mullerian duct formation during embryonic development. Unusual Mullerian duct anomalies are didelphys uterus, which sometimes indicates "double uterus." A rare condition is a didelphic uterus with ipsilateral renal agenesis and blocked hemivagina. It typically manifests as dysmenorrhea, pelvic pain after menarche, and a palpable lump or increase in abdominal volume as a result of unilateral haematocolpos. The diagnosis was ipsilateral renal agenesia, uterine didelphys, and blocked hemivagina. To diagnose and cure the condition, a laparotomy was done. On the right side were two cervices with hematometra and hematocolpos, two separated hemiuteri, and ipsilateral renal agenesis. Strassman metroplasty was done after the vaginal septum was entirely removed. Diagnosing and treating this congenital disability can be difficult because of the heterogenic appearance, vague complaints, and intricacy of the anatomic tissues. When working up female patients who appear with episodic abdominal discomfort and an abdominopelvic mass, these abnormalities must always be taken into account.
