Abstract

Immunization remains one of the most impactful public health interventions, significantly reducing morbidity and mortality from vaccine-preventable diseases. India, with the world’s largest immunization program by volume, has witnessed substantial progress in vaccine coverage, integration of technology, and policy innovation. This review explores the evolution of immunization practices in India, tracing developments from historical foundations to the modern era, including the Universal Immunization Program (UIP), responses to the COVID-19 pandemic, and the adoption of digital platforms like Co-WIN and eVIN. It highlights disparities in coverage across states, challenges such as vaccine hesitancy and weak surveillance, and strategies for overcoming these barriers. The document also introduces the life course immunization approach, advocating for vaccinations beyond childhood to ensure comprehensive protection throughout life. With advancements in medical science and public health infrastructure, India is poised to leverage immunization as a cornerstone of its national health agenda, aligning with the goals of the Immunization Agenda 2030 (IA2030) and Sustainable Development Goals (SDGs).
