Abstract

Medication nonadherence is a major challenge in psychiatric care, leading to poor clinical outcomes, frequent relapses, increased hospitalizations, and a greater burden on healthcare systems and families. Despite effective treatments, many patients fail to take medications as prescribed due to a combination of psychological, social, and systemic factors. Stigma surrounding mental illness, unpleasant side effects like weight gain or drowsiness, and lack of insight into one’s condition often hinder adherence. For instance, individuals with schizophrenia may stop medication once symptoms improve, while those with depression may lack motivation or energy to continue treatment. Socioeconomic barriers such as financial constraints, limited healthcare access, and inadequate social support further complicate adherence, especially in rural or low-resource settings. Additionally, fragmented healthcare services and poor patient education can leave individuals uninformed or disengaged from their treatment plans. The consequences are far-reaching  symptom relapse, worsening of illness, hospitalization, and even suicide risk increase when adherence falters. Families face emotional and financial strain, and the healthcare system bears a higher economic burden due to recurrent acute care needs. Addressing this issue requires empathy and collaboration. Strengthening the therapeutic alliance, providing psychoeducation, simplifying medication regimens, and using long-acting injectables or digital reminders can enhance adherence. Involving families and peer support networks also fosters continuity and motivation. Ultimately, promoting medication adherence goes beyond compliance it is about building trust, empowerment, and hope, enabling individuals to actively participate in their recovery and improve their overall quality of life.
